Forms and Registration

These forms are available on this website, or at your first visit to The Ridge Street Country School.

These forms need to be filled out and promptly returned to our office, where they will be kept on file. The information you supply on these forms affects your child's well-being while at school and is helpful to your child's teachers, as well as to our administrative staff in case of an emergency.

The Medical Information form gives us the necessary immunization information that is a New York State requirement. Medical forms need to be submitted on a yearly basis. As your child's medical form expires and you complete your next annual visit to the doctor, please be sure to send in your updated form.

The Emergency Medical Treatment Authorization form must be notarized with the emergency contact numbers kept current. It is imperative that you keep the school informed of up-to-date phone numbers, emergency numbers and other pertinent information.
